We know some of the documents you need are not always provided to you. We are here to help. Below are links to the California Secratery of State's website so that you can download and print an Acknowledgement and Jurat. Don't worry though we always have extra copies with us when we meet with you.
The purpose of an acknowledgment is for a signer, whose identity has been verified, to declare to a Notary or notarial officer that he or she willingly signed the document.

This acknowledgment wording is mandatory for all acknowledgments taken in the state.
Jurats are the second most commonly used notarial act.
This act requires a notary to administer an oath or affirmation from the signer. This oath or affirmation determines the signer agrees the document is true under penalty of perjury. Therefore, with the signer signing and completing the swearing of the document, it allows a notary to complete their notarial act.
